REDMOND SCHOOL DISTRICT DOES NOT PROVIDE STUDENT ACCIDENT INSURANCE.

However, you may purchase this coverage for school accidents through a special program offered to the district.

Note that all athletics EXCEPT GRADES 10-12 FOOTBALL are covered automatically if you purchase the 24-HOUR COVERAGE or the AT-SCHOOL COVERAGE. However, purchasing FOOTBALL COVERAGE alone will only cover grades 10-12 football accidents. (Any 9^th grade student that plays with the senior team must purchase senior high school coverage.) You may need the 24-HOUR COVERAGE or the AT-SCHOOL COVERAGE as well if you have a multi-sport athlete. In addition, you may purchase optional EXTENDED DENTAL COVERAGE.

Complete the online application print a copy of the confirmation for your records. A copy of this same confirmation of your new insurance must be received at the school office. If you do not wish to purchase student insurance, please complete the form below and return it to the school office.

The Redmond School District endorses this plan but it is not responsible for the administration of the plan. The contract is between you and the insurance carrier. The insurance agent, Western States Insurance (541-475-2249), is authorized to assist you with any questions or problems you may have regarding student accident insurance.
