Dental Screening is Scheduled on November 17, 2025

Students will only be screened and services provided by Advantage Dental if their parent/guardian checked the yes box on their registration form for dental screening. Please contact the school office with any questions, concerns, or if you would like to fill out paperwork to opt your child in for screening and/or other services.

Immunization Note
Our school district requires complete or up-to-date immunization records. Please take your child to a pharmacy, clinic or to your family doctor to get updated on the immunizations if needed or complete the non-medical immunization exemption online. -All students are required to have an updated immunization record on file.
-7th grade students are due for a Tdap. 

According to Oregon law, your 7th grade child is required to have one additional vaccine. An official medical or nonmedical exemption is also acceptable. 

Tdap Vaccine – 1 dose 

• Tdap is given to adolescents and adults 10-64 years old. 
• It is different from DTaP, which is given to children younger than 7 years old.

Tdap offers adolescents and adults protection against three serious diseases: Tetanus, Diphtheria, and Pertussis (whooping cough). Pertussis is a highly contagious, acute bacterial respiratory tract infection caused by the bacterium Bordetella pertussis. 

If your child has not yet had the Tdap vaccine, contact your child’s doctor or call your county health department to make an appointment. It is important to get an early start on updating your child’s immunizations this fall. Please give all updated immunization information to the OMS office as soon as your child receives Tdap.

Bring your completed non-medical exemption form or your child's updated immunization record to school after they have received the required immunizations to prevent your child from being excluded from school and all school related activities/programs on Exclusion Day, 2/25/2026.
